Medical Leave Process
Login to MyUP
Click on menu on left-hand side
Click employee
Click ehealthsafe
Select request Medical Leave of Absence
Select dates you need off (must be at least 4 days) and enter the reason for the leave in the box.
Click Submit in bottom right
Select messages on the toolbar on the left-hand side
Upload doctors note (may need to be in PDF format)
Paperwork should arrive by mail within 14 days. However, check the attachment button in ehealthsafe on the "to do list." Generally, for longer requests, the paperwork will be mailed but it's only available online for shorter MLOA's
