PS Day Documentation Request
In accordance with the above stipulation within the Time off for Illness and Wellness Agreement, documentation can only be requested if two PS days are used consecutively or PS is used in conjunction with other leave.
They can ask for an explanation of why you used a PS day without requesting documentation.
If the carrier requests documentation for the use of consecutive PS days or the use of a PS day in conjunction with other leave:
Make sure you complete the TRM ticket process they outline or they will pursue failure to comply charges. Generally, they only give 48 hours get the TRM ticket completed after reading the message.
Make sure you upload your documentation to the TRM ticket.
If the carrier requests documentation for a PS day and it was only a single PS day and not used in conjunction with other leave:

On the TRM ticket, use the template below with the following information and then state why you took the PS day (e.g. wasn't feeling well so I took OTC medications and got extra sleep, etc.):
"At the time of my layoff, I was working under the SMART-TD Time off for Illness and Wellness Agreement. Article III, Section D of this agreement states documentation can only be requested for the use of two consecutive PS days or a PS day used in conjunction with other leave. Documentation is not required for the use of a single PS day.
However, I utilized a PS day on ____(date)____ for ________( brief explanation)_________."
