• Images of empty boxes, pallets, and/or refrigerator.

REQUIRED CLAIM LANGUAGE:

Claiming basic day account Item 81 violation. Called on duty at _____ hours on the __train__  __ at wx___.  At ____ hours on __/__/__, I was informed by the inbound crew that we needed to bring _____________ when we recrew. After receiving this information, I was unable to locate any _____________ inside the depot. This issue has also been reported to manager _____________. Claiming basic day for not having the necessary supplies required by Item 81.
