YOUR CLAIM:
When a PS day is taken or "burned" when laying off for FMLA (FL status).
REQUIRED DOCUMENTATION:
Proof of layoff in FMLA status.
A copy of your timeslip showing payment of PS day on the date FMLA was taken.
REQUIRED CLAIM LANGUAGE:
"Claiming a basic day penalty account on (DATE) I was forced to use a paid sick day concurrently with my FMLA layoff."
